What are ginsenosides and how does ginseng work?

The Cochrane review by Geng and colleagues (2010) concluded there was a lack of convincing evidence that Panax ginseng enhances cognition in healthy people, and no high-quality evidence in dementia [3]. A newer meta-analysis by Zeng and colleagues (2024) found a significant effect on memory specifically, but no benefit for overall cognition, attention or executive function [4]. So the honest cognitive claim is narrow: possibly memory, not general brain-boosting.

Erectile function is the area with the most eye-catching signal. Jang and colleagues (2008) reviewed seven trials of red ginseng and found a significant effect versus placebo, but stated plainly that the number and quality of trials were too low for definitive conclusions [5]. Suggestive, not settled.

Blood sugar is both a modest benefit and a safety flag. Shishtar and colleagues (2014) pooled 16 trials and found ginseng modestly but significantly improved fasting blood glucose, without significant effects on HbA1c or insulin resistance overall [6]. That glucose-lowering direction is what creates the interaction caution with diabetes medication.

Where the evidence is negative or preliminary: athletic performance research mostly shows no benefit, and the flu-prevention signal is preliminary. The overall grade is limited, and the widely repeated framing of ginseng as a clinically proven energiser overstates what the trials support.

How should you take ginseng?

If you want to try ginseng as a gentle tonic for fatigue and weakness, keep expectations calibrated to the evidence and use it sensibly.

Dose: European traditional-use guidance gives ranges including 0.6 to 2g of powdered root daily, 40 to 200mg of a standardised dry extract (with up to 600mg in the first few days in some cases), or roughly 1.2 to 1.8g of red ginseng, for up to three months. Match the dose to the standardisation stated on the label rather than to the raw milligram figure.

Timing: take it in the morning. Ginseng's most common side effect is insomnia, so evening dosing invites trouble.

Duration: the traditional framework caps continuous use at around three months, and advises a medical review if the symptoms you are treating persist beyond a couple of weeks. Cycling rather than indefinite daily use is the conservative default.

Who should not take ginseng?
