Magnesium L-Threonate · mineral
Evidence tier: limited
A magnesium form designed to reach the brain, for cognitive symptoms and anxiety where brain magnesium matters most.
The threonate carrier is claimed to ferry magnesium across the blood brain barrier more effectively than other magnesium salts, raising magnesium in the brain and thereby increasing synaptic density. This was demonstrated in rodents. The step that has never been shown in people is the one the whole product depends on: that oral magnesium L-threonate raises human brain magnesium enough to change cognition. Treat it as an unverified extrapolation from animal work.
Limited. Human trials do exist, so this is not quite an evidence vacuum, but every one of them is small, and every one of them traces back to the company that sells the branded ingredient. The cognitive reputation of magnesium L-threonate was built on RODENT work showing that this particular salt raises magnesium concentrations in the brain, and the marketing has been running ahead of the human data ever since. The human record amounts to: a 2016 pilot in 44 older adults with mild memory complaints, industry linked; a 2024 randomised trial in 80 adults with self reported sleep problems; and a 2026 randomised trial in 100 adults aged 18 to 45 with self reported dissatisfied sleep (PMID 41601871), which reported greater improvement on the NIH Total Cognition Composite at p equal to 0.043. That last figure deserves to be read carefully rather than repeated. It is a borderline p value on a composite score, in a 6 week trial, funded by Threotech Inc, which supplied the product, with a lead author who discloses research funding and honoraria from nutraceutical companies. In the same trial the objective sleep tracking showed NO group difference and one cognitive test, Raven's Progressive Matrices, showed no difference either. The trial's headline claim of a 7.5 year reduction in brain cognitive age is a derived, marketing friendly restatement of a modest composite shift, not an independent finding. There is no independent, non industry replication of any cognitive outcome, and the older adult trial that is cited most often was conducted in people with mild cognitive impairment, a clinical population.
1 to 2 g daily of magnesium L-threonate in the human trials, which is the compound weight and not the elemental magnesium weight. The elemental magnesium delivered is a small fraction of that, so this is an expensive way to obtain magnesium and it should not be assumed to cover a general magnesium requirement.
1.5 to 2 g daily (provides ~144 mg elemental magnesium), in evening dose.
Generally safe. Caution in renal impairment.
