Evidence tier: moderate

In plain terms

Reduces chronic inflammation and supports joint health

How it works

Curcumin is thought to damp down the body's main inflammatory switch, a signalling protein called NF-kB, which reduces the output of inflammatory messengers. This is the leading laboratory explanation for the joint pain effect, supported in cell and animal work. It is plausible, not proven to be the operating mechanism in people at the doses they take. Turmeric separately stimulates bile flow, a mechanism the EMA accepts well enough to write a contraindication around it.

What the evidence says

Moderate for knee osteoarthritis pain, and that is the only defensible use. Start with the authority: NCCIH, the US National Center for Complementary and Integrative Health, concludes that we do not know enough to definitively conclude if turmeric or curcumin is beneficial for any health purposes. The knee osteoarthritis literature is large and consistently positive in direction (a 2024 umbrella review of eleven meta analyses, PMID 38576215; a fifteen trial review in 1,621 people, PMID 34017975; a Bayesian network meta analysis, PMID 38036015), but the best and newest pooling, a 2025 network meta analysis of seventeen trials, states that the certainty of evidence remains low, and only the absorption enhanced preparation cleared the threshold of a difference a patient would actually notice (PMID 40731001). When seven joint supplements were ranked head to head in 2025, Boswellia, not curcumin, came out on top (PMID 40806131). Fatty liver and mouth ulcers from cancer treatment are limited. Digestion is traditional use only: Europe's regulator, the EMA HMPC, registers turmeric as a traditional herbal medicine for digestive disturbances resting exclusively upon long standing use, not on trial evidence. Everything else, including blood sugar, insulin resistance, skin, autoimmune flares, muscle recovery and systemic inflammation, is insufficient. Safety is the other half of the correction: NIH LiverTox rates turmeric a likelihood score A, the top of its scale, and calls it the most common cause of clinically apparent herb related liver injury in the United States.

Dose and timing

Two different products with two different dosing worlds. As a traditional herbal medicine (EMA registered): 0.5 to 1 g powdered turmeric, two to three times daily. As a supplement extract: doses in circulation run from 100 mg to over 1,000 mg daily depending on formulation. Take with food. NCCIH considers conventionally formulated turmeric or curcumin, meaning not modified to boost absorption, likely safe in recommended amounts for up to two or three months, and does not extend that reassurance to high absorption versions.

Curcumin bioavailability is low (6 to 7% from turmeric powder alone). Always combine with piperine (black pepper, 5 to 10mg) or consume with fat (olive oil, coconut oil, full fat dairy) to increase absorption 2000 fold. Enteric coated or liposomal formulations further enhance bioavailability. Typical lag time to efficacy: 4 to 8 weeks.

Safety

Excellent safety at food level intake and standard supplemental doses. Culinary doses safe throughout pregnancy. High dose extracts (>3g daily) best avoided in pregnancy. May cause mild GI upset; take with food. Bioavailability poor without piperine or fat co ingestion.

Pregnancy: avoid

Breastfeeding: avoid

Traditional use

Used for over 4,000 years in Ayurvedic and Traditional Chinese Medicine as an anti inflammatory, digestive bitter, and wound healer. Called the golden spice of India. Curcumin, its active compound, is one of the most extensively studied plant molecules in modern botanical research.

Active compounds: curcumin, demethoxycurcumin, bisdemethoxycurcumin
