Ashwagandha root extract is an adaptogen with some of the strongest clinical evidence for male reproductive health in herbal medicine. Multiple RCTs show improvements in testosterone, sperm count and motility, muscle strength, and exercise recovery. The mechanism involves HPA axis modulation (cortisol reduction) combined with direct support of gonadal function. KSM-66 extract (root-only, 5% withanolides) is the most studied and bioavailable form.
1. Take 600mg KSM-66 ashwagandha daily — either as a single morning dose or split 300mg morning and 300mg evening with meals. 2. Take consistently for at least 8 weeks. Testosterone and cortisol changes begin within 4 weeks but measurable reproductive improvements require 3 months minimum. 3. For fertility goals (sperm count/motility): commit to 90 days. Spermatogenesis takes ~74 days — improvements take time to manifest. 4. If you have thyroid disease or are on thyroid medication, inform your doctor before starting — ashwagandha may increase thyroid hormone levels. 5. Pair with resistance training for maximum muscle strength and testosterone benefit — the Wankhede 2015 RCT showed significantly greater gains when combined with an exercise programme.
Ashwagandha withanolides modulate the hypothalamic-pituitary-adrenal (HPA) axis, reducing cortisol by 27-28% (Chandrasekhar 2012 RCT). Chronic cortisol elevation suppresses testosterone via gonadotropin inhibition, cortisol reduction restores LH and FSH pulsatility. Ambiye VR et al. (2013) RCT (n=46, oligospermic men): KSM-66 675mg/day for 90 days increased testosterone 17%, sperm count 167%, motility 57%, semen volume 53%, and serum LH vs placebo. Wankhede S et al. (2015) RCT (n=57, healthy males): ashwagandha significantly improved 1RM bench press (+46kg vs +26kg placebo), muscle recovery (serum creatine kinase reduction), and VO2max.
