How to prepare

1. SCREENING REQUIRED: Must be cleared for (1) no personal history or high risk of hormone-sensitive cancer (breast, uterine, ovarian), (2) no existing liver disease, (3) not on tamoxifen or other SERMs. 2. Take one standardised tablet twice daily with meals. Most preparations provide 20 mg iCR extract per tablet. 3. Allow 4–8 weeks for full effect on hot flush frequency and intensity. 4. Monitor for any yellowing of eyes/skin, dark urine, or upper right abdominal pain — stop immediately and seek medical review (signs of hepatotoxicity). 5. Do not use beyond 6 months without reassessment with a healthcare provider.

Why it works

Black cohosh (Actaea racemosa) contains triterpene glycosides and phenolic compounds with SERM (selective oestrogen receptor modulator) activity and serotonergic effects on hypothalamic thermoregulation. Osmers R et al. (2005) RCT (n=304) found iCR extract significantly reduced Kupperman index score vs placebo. Leach MJ & Moore V (2012) Cochrane review (16 RCTs) found inconsistent but generally positive evidence for menopausal symptom reduction. Hepatotoxicity risk: WHO Pharmaceuticals Newsletter and multiple regulatory agencies have documented rare but serious liver injury, mechanism unclear, possibly immune-mediated. Estimated incidence extremely rare but warrants hepatic monitoring.
