Boswellia is the resin of Boswellia serrata, the frankincense tree, and has been used in Ayurvedic practice for centuries to support comfortable movement. Research interest centres on the 5-lipoxygenase pathway, which is a different one from the enzymes most familiar painkillers act on. That makes it scientifically interesting; it does not make it a replacement for anything you have been prescribed.

How to prepare

1. Choose a product standardised to AKBA (11-keto-beta-boswellic acid) content — this is the most potent anti-inflammatory compound in boswellia. Look for '30% AKBA' or a named extract (Aflapin, 5-Loxin). 2. Take with a fatty meal (avocado, olive oil, nuts) — fat significantly improves AKBA absorption. 3. Allow 4–6 weeks for measurable pain and stiffness reduction. Some people notice earlier improvement. 4. Can be used alongside NSAIDs or with the goal of reducing NSAID dependence under guidance — the mechanisms are complementary, not duplicative. 5. Long-term use (6+ months) is generally well-tolerated — boswellia lacks the GI ulceration risk associated with chronic NSAID use.

Why it works

Acetyl-11-keto-beta-boswellic acid (AKBA) is the most potent anti-inflammatory boswellic acid. It selectively inhibits 5-lipoxygenase (5-LOX), the enzyme that converts arachidonic acid to pro-inflammatory leukotrienes (LTB4, LTC4, LTD4). AKBA also inhibits microsomal prostaglandin E synthase-1 (mPGES-1) and cathepsin G, reducing cartilage matrix degradation. Kimmatkar N et al. (2003) RCT (n=30): AKBA significantly reduced knee OA pain (p<0.001), swelling, and improved knee flexion vs placebo. Sengupta K et al. (2011) RCT: Aflapin (100 mg) showed significant improvement in pain and function vs placebo within 30 days.
