How to prepare

1. CHECK FIRST. Is the swelling in ONE leg only, and is that leg painful, hot, red or tender in the calf? If so, stop. That is a suspected deep vein thrombosis and it needs urgent medical assessment today, not a soak. 2. Run cool water, around 15 to 20°C, deep enough to cover the calves. Cool, not ice cold. 3. Immerse the lower legs for 10 to 15 minutes. If using a shower instead, run cool water over each leg from the ankle upward toward the knee for a couple of minutes per leg. 4. While immersed, pump the ankles: point the toes down, then pull them firmly up toward the shin. Twenty repetitions, then rest, then repeat twice more. This is what reactivates the calf pump and it is doing as much work as the water is. 5. Get out, dry the legs, and lie down with the legs raised ABOVE HEART LEVEL on stacked pillows for 15 to 20 minutes. Above the heart is the specification. A footstool is not high enough. 6. Keep doing ankle pumps while elevated. Elevation plus movement clears far more fluid than elevation alone. 7. If you use compression socks, put them on first thing in the morning before the legs have had a chance to swell, not in the evening after they already have. 8. Across the day, break up long periods of standing or sitting. Even standing on tiptoes twenty times every hour engages the calf pump. 9. Do NOT add Epsom salts expecting a magnesium effect. Transdermal magnesium absorption is not supported by evidence. The cool water and the elevation are doing the work. 10. See a doctor if swelling is persistent, affects both legs and is new, or comes with breathlessness, which can indicate a heart, kidney or liver cause rather than a local venous one.

Why it works

Blood and fluid pool in your legs when the calf muscle pump stops working. Cool water tightens the surface veins, ankle pumping restarts the pump, and raising the legs above your heart lets gravity drain them instead of filling them.

Time to effect

This is symptomatic relief. It does not repair incompetent venous valves. Persistent or worsening swelling needs a medical assessment and possibly a vascular referral.

Not right for

Not for new swelling in one leg with pain, warmth or redness, which is a suspected blood clot and needs urgent care today. Not for new swelling in both legs with breathlessness, which can be a heart, kidney or liver problem. Not for anyone with peripheral arterial disease using compression without medical advice. Not for sudden swelling in pregnancy, which needs urgent maternity assessment.

What the evidence says

The measures in this record are physical: cool immersion, elevation above heart level, calf pumping, and graduated compression at 10 to 20 mmHg, which has moderately robust support for occupational leg symptoms and venous oedema. The botanical option with real evidence, horse chestnut, is a standardised capsule and its raw seeds are toxic, so there is no home preparation of it.

Who it suits best

Best for someone whose legs ache and swell predictably after a long day standing or sitting, with swelling in both legs that resolves overnight, no one sided pain or redness, and no arterial disease. If the swelling is new, one sided, or does not settle overnight, see a doctor first.
