Coenzyme Q10 (CoQ10) is a fat-soluble compound essential to mitochondrial electron transport chain function, every cell uses it to generate ATP. Production declines with age and is significantly suppressed by statin medications (same mevalonate pathway). Two forms: ubiquinone (oxidised, cheaper) and ubiquinol (reduced, more bioavailable). Most evidence is in cardiovascular disease and statin-induced myopathy, where depletion is documented and supplementation is mechanistically compelling.
1. Take 100-300mg CoQ10 with your largest meal of the day — fat significantly improves absorption. Breakfast or dinner with food works well. 2. Choose ubiquinol over ubiquinone if you are over 40, on statin medication, or have cardiovascular disease — it is the reduced, active form and is more bioavailable. 3. For statin-induced muscle fatigue: start CoQ10 at 200mg daily and give it 4-8 weeks. Energy improvement is gradual. 4. If you take warfarin, monitor your INR after starting CoQ10 — it has structural similarity to vitamin K and may slightly reduce warfarin's anticoagulant effect. 5. Track your energy levels weekly — improvements in cognitive energy and exercise tolerance are often reported before objective physical improvements.
CoQ10 (ubiquinone/ubiquinol) functions as an electron carrier in mitochondrial complexes I, II, and III, essential for oxidative phosphorylation and ATP synthesis. It also acts as a lipid-soluble antioxidant protecting cell membranes. Statins inhibit HMG-CoA reductase, blocking mevalonate, the precursor for both cholesterol AND CoQ10. Rosenfeldt FL (2007) meta-analysis (12 trials): CoQ10 reduced systolic BP by up to 17mmHg in hypertension. Myopathy evidence from multiple observational studies is consistent.
