Cranberry contains type-A proanthocyanidins (PAC-A), compounds that block the fimbriae (hair-like adhesion appendages) of uropathogenic E. coli from attaching to the urinary tract lining. Without adhesion, bacteria are washed out during urination. The evidence is specifically for PREVENTION of recurrent UTI, not treatment of an active infection. Any active UTI with systemic symptoms (fever, back pain) requires antibiotic treatment.

How to prepare

1. Choose a cranberry supplement that clearly states PAC-A content on the label. 36mg PAC-A per day is the minimum effective dose — many products are significantly underdosed. 2. Take daily, consistently. Cranberry works preventively by keeping the urinary tract inhospitable to bacterial adhesion — it must be maintained. 3. If using juice rather than capsules: drink 240-300ml of unsweetened cranberry juice daily. Avoid cranberry cocktail products — they have very low PAC-A and high sugar. 4. Do NOT use cranberry as the sole treatment for an active UTI — it does not have sufficient antimicrobial activity to clear infection. See a doctor if you have burning, urgency, or fever. 5. If you take warfarin (blood thinner), check your INR more frequently when starting — cranberry may increase its effect. 6. Combine with good hydration (2-3L water daily) for maximum preventive effect.

Why it works

Cranberry type-A proanthocyanidins (PAC-A) are structurally unique, the A-type linkage is the key feature that blocks P-fimbriae and type-1 fimbriae on uropathogenic E. coli. These fimbrial adhesins are essential for initial colonisation of the urinary epithelium. Without adhesion, bacteria cannot establish infection and are cleared during voiding. Jepson RG et al. (2012) Cochrane review of 24 RCTs (n=4,473): cranberry products significantly reduced UTI recurrence vs water/placebo, particularly in women with recurrent infections (RR 0.74, 95% CI 0.60-0.91). NNT approximately 7 women/year to prevent one UTI.

Not right for
