D-mannose works by flooding the urine with mannose sugar molecules that E. coli, responsible for ~80% of uncomplicated UTIs, prefer to bind to over urinary epithelium. Bacteria are literally swept out in urine rather than establishing infection. Uniquely, this mechanism does not drive antibiotic resistance and is safe for almost everyone. Most effective for E. coli UTI, less effective for other organisms.
1. ACUTE UTI: Dissolve 2g D-mannose powder in a large glass of water (250-300ml). Drink this every 2-3 hours for the first 3 days — this maintains high mannose concentration in urine throughout the day. 2. After 3 days (acute phase): reduce to 2g once or twice daily for a further 7-10 days to ensure complete clearance. 3. For PREVENTION: 2g daily in water, long-term. This maintains sufficient urinary mannose concentration to prevent E. coli adhesion. 4. If symptoms worsen, spread beyond the bladder (fever, back pain, rigors, vomiting), or do not improve within 48 hours — seek urgent medical care. D-mannose does not treat kidney infections (pyelonephritis). 5. D-mannose is specific to E. coli (the most common UTI cause). If your infections are caused by other bacteria (Klebsiella, Proteus, Enterococcus), D-mannose will be less effective. 6. If you have diabetes, note that D-mannose is metabolised differently to glucose but does affect blood sugar at high doses — monitor accordingly.
D-mannose binds to type-1 fimbriae mannose-binding lectins (FimH adhesins) on uropathogenic E. coli. When high concentrations of free D-mannose are present in urine, E. coli preferentially bind the soluble sugar rather than the mannosylated glycoproteins on uroepithelial cells. Bacterium-mannose complexes are then washed out during urination. Kranjčec B et al. (2014) RCT n=308 women with recurrent UTI: 6-month follow-up: D-mannose 2g daily → 14.6% UTI recurrence vs 60.8% no treatment vs 20.9% nitrofurantoin 50mg. No antibiotic resistance, a critical advantage given rising UTI antibiotic resistance rates globally.

D-Mannose is a simple sugar that E. coli bacteria (the cause of most UTIs) cling to instead of your bladder wall, the bacteria latch onto the mannose and get washed out in urine. It is specific, effective, and very well tolerated.
