How to prepare

1. Choose a product standardised to 70–80% silymarin content. The label should show silymarin content, not just herb weight. 2. Take 140 mg silymarin with each meal (up to 3× daily = 420 mg), or follow product labelling for your dose. 3. Swallow with a full glass of water. Taking with meals reduces the mild GI upset some people experience. 4. Use consistently for at least 4–8 weeks to assess benefit. Liver enzymes typically show improvement within 4–12 weeks if responding. 5. Do not use as a substitute for medical treatment if you have known liver disease — use alongside, not instead of, medical care.

Why it works

Silymarin is a flavonolignan complex (silybin, silydianin, silychristin) with four complementary hepatoprotective mechanisms. Ferenci P et al. (1989) randomised 170 patients with alcoholic cirrhosis to silymarin or placebo and found significantly improved survival and liver function tests in the silymarin group. Mulrow C et al. (2000) systematic review of 13 trials found milk thistle improved liver histology and aminotransferases across multiple aetiologies. Importantly, silymarin has poor water solubility, tea preparations are largely ineffective, and standardised capsule preparations are required.
