Omega-3 fatty acids work via a fundamentally different mechanism to most anti-inflammatories, they don't block enzymes but instead replace the substrate that drives inflammation. By incorporating into cell membranes in place of arachidonic acid, EPA and DHA shift the body's eicosanoid production towards less inflammatory prostaglandins and leukotrienes. This is particularly effective for systemic inflammatory conditions like rheumatoid arthritis.
1. Check the label carefully: look for total EPA+DHA content, not total fish oil weight. You need 2–4 g combined EPA+DHA daily for anti-inflammatory effect. 2. Take with meals — fat-rich meals improve omega-3 absorption. Divide dose across 2 meals (e.g., 1–2 g at lunch and 1–2 g at dinner). 3. Enteric-coated capsules reduce fish burps. Freeze capsules to further reduce GI side effects. 4. Allow 8–12 weeks for measurable joint symptom improvement — membrane remodelling takes time. 5. If on warfarin or blood thinners: consult your pharmacist before taking high-dose omega-3 (≥3 g/day). Clinically significant at high doses.
EPA and DHA competitively replace arachidonic acid in phospholipid membranes. When inflammatory signals trigger phospholipase A2, EPA/DHA-derived eicosanoids (PGE3, LTB5) are far less pro-inflammatory than arachidonic acid-derived PGE2 and LTB4. EPA additionally generates resolvins and protectins, lipid mediators that actively resolve inflammation. Goldberg RJ & Katz J (2007) meta-analysis (17 RCTs) found omega-3 significantly reduced joint pain intensity (WMD -0.31, p<0.001) and morning stiffness duration. Effect was most pronounced at ≥2 g EPA+DHA per day with ≥3 months of use.
