Saw palmetto (Serenoa repens) is the most widely used botanical for BPH, estimated to be taken by over 2 million men in the US. A 1998 Cochrane review of 18 RCTs (n=2,939) found saw palmetto improved urinary symptoms comparably to finasteride. Inhibits 5-alpha reductase (reducing DHT) and modulates androgen receptors in prostatic tissue. Full benefit typically emerges after 3-6 months of consistent use.
1. Take 320mg standardised extract once daily with your largest meal, or split into 160mg twice daily (morning and evening). 2. Be patient — this is a slow-acting remedy. Most men notice improvement in urinary symptoms after 4-8 weeks, with full benefit at 3-6 months. 3. If you are scheduled for a PSA (prostate-specific antigen) test, inform your doctor you are taking saw palmetto — it can reduce PSA by up to 50%, which affects interpretation. 4. If you are already on finasteride or dutasteride (Proscar, Avodart), discuss with your prescriber before adding saw palmetto — both inhibit 5-alpha reductase. 5. Track your night-time urination episodes weekly — this is the most objective self-report measure. Improvement should be measurable by week 6-8. 6. Symptoms of BPH (especially weak stream) can also indicate prostate cancer — ensure you have had appropriate medical screening if you have not already.
Saw palmetto fatty acids and phytosterols inhibit 5-alpha reductase isoenzymes types 1 and 2, reducing conversion of testosterone to dihydrotestosterone (DHT). DHT is the primary driver of prostatic epithelial proliferation in BPH. Saw palmetto also inhibits DHT binding to androgen receptors in prostate cells and reduces COX-2 mediated prostatic inflammation. Wilt et al. (1998) Cochrane review of 18 RCTs (n=2,939): saw palmetto improved urinary flow rate by 28% vs placebo and reduced nocturia comparably to finasteride, with fewer sexual side effects. Notably, the large STEP trial (Barry 2011, n=369) found saw palmetto equivalent to placebo over 18 months, indicating response is not universal and extract standardisation matters significantly.
